*Download the file and click "Enable Editing" – Open in Excel, not Google Sheets

*Complete the form and save it as "Spiritwear-(your swimmers names)"

*Email the form to [email protected]

*Pay for your order via Venmo to Betsy-Allen-9, or by check to Tavistock Tsunamis
